Wards Updates

VA Updates:
VA is expanding the diagnoses that IM-led stepdown level of care can take - any cardiac case that is HDS, not on a titratable drip, or in cardiogenic shock/needing invasive monitoring (pacing, swan, etc) can be admitted to medicine for stepdown!

UC Updates:
In conjunction with eCART, you still may use Watcher Status order (q24h) for tenuous patients who potentially may worsen and need higher level of care, regardless of eCART score. This is for physicians (in handoff), for nurses (W placed on door, discussed in huddle), and for the STAT RN to help evaluate if needed - if you want them to round/evaluate, make sure to call them (584-9110), if not can comment “no need for STAT RN” in order comments.

Board-Style Question

A 22-year-old man presents to the emergency department in late summer with a 2-day history of headache, fever, and photophobia. He reports mild neck stiffness but no confusion or focal neurological deficits. He is otherwise healthy and takes no medications. He attends college in Wisconsin and lives in a dormitory.

On examination, temperature is 38.2 °C (100.8 °F), heart rate is 88/min, and blood pressure is 118/72 mm Hg. He is alert and oriented. No rash is present.

Cerebrospinal fluid analysis reveals:

• Leukocyte count: 180/µL (predominantly lymphocytes)

• Glucose: 65 mg/dL (normal)

• Protein: 48 mg/dL (mildly elevated)

• Gram stain: Negative

• Bacterial cultures: Pending

Answer:

The answer is D) Enterovirus. Enteroviruses are the leading cause of viral (aseptic) meningitis in healthy young adults, especially during summer and early fall, and are characterized by a CSF profile of lymphocytic pleocytosis, normal glucose, and mildly elevated protein. This epidemiology and clinical presentation are supported by multiple cohort studies and reviews in the medical literature. The Infectious Diseases Society of America and the American Society for Microbiology also identify enteroviruses as the most common etiologic agents of acute viral meningitis in immunocompetent adults in the United States. Other viruses such as HSV-2 and VZV are less common in this age group and clinical context.
